Your interest and chosen field seems a bit contradictory.
You mentioned dreaming of becoming a doctor, but looks like you have applied for in the engineering stream.
So my question right now is whether you dream of being a doctor or an engineer?
As such, if you have not appeared for Medical entrance exam, now the time is out, so you would have to wait for another year and then try your luck at the medical entrance exam. In the meanwhile probably you could take admission in B.Sc, or engineering, as you have already secured a seat there. But note, that, it would be difficult to concentrate on preparing for the entrance test, if you take up engineering.
Secondly, since you have appeared for the engineering entrance test and have secured admission into the engineering college, which clearly means that you dream of being an engineer. Please note that there is a possibility that you have another dream which is way apart from what your family wishes for. But you need to be sure about it and it should not mean that you are being adamant and do not wish to go their way.
In-case, you dream of engineering, I would suggest secure admission in the college you have got selected into. Rather with all your cool and without arguments, try convincing your family about your dream and how you intend to achieve it. Please note that, when you have an interest in another field, pursuing a course because of family demands, you may not be successful in that course and pursuing the course haphazardly will not secure a future for you.
As far as the college ranking is concerned, as long as the college has good placements, your job is done. Please note, the market focuses on the students calibre and then on the ranking of the college you have studied or graduated from. You may be from the top college, but if you have no calibre, no big names would be of any support to you.
While studying focus on being more practical and not just focusing on the lectures. Opt for internships and project work to enhance your knowledge that will take you a long way.
No looking back should be your motto.

By the tone of your concern, I definitely feel that you neither wished to become a doctor nor you have interest in the medical field. If it was, then you would have appeared for the Medical Entrance Exam and not the Engineering Entrance examinations.
Further you have got a seat in an engineering college but you have not liked that college. This means that your score in engineering entrance test was also not above average and therefore you have got an average engineering college seat.
I really feel, that you are not clear and do not know what course you need to do. Engineering nor Medical seems to be your interest and choice.
